In a promo for Rey Mysterio's WWE debut on "SmackDown" in 2002, he can be seen doing pull-ups on a shelf in the background while Edge and John Cena formulate a game plan. However, the board broke, causing him to fall dramatically, and the wrestlers and crew can be heard erupting with laughter as he lay on the ground in stitches.

When interviewer Greg Hamilton asked Kalisto what he'll do next, he promised to "shock the world," vowing to "make a Good Lucha Thing" and giving a hearty "Woo-hoo!" before walking off. On "Superstars" in 2010, Matt Hardy fell over the ropes while doing his signature V1 pose. The mistake has been edited out of the cut, available on YouTube, with Hardy still sporting the same huge grin of embarrassment, and Josh Matthews said, "Ironically this video was a metaphor for Matt Hardy's career."

The 2005 Royal Rumble was intended to close out with Batista eliminating John Cena, but instead, both fell over the ropes and hit the floor in a photo-finish tie. Vince McMahon charged the ring, tearing both of his quads, and while sitting in immense pain, McMahon confided with referees, calling for the match to be restarted.
